The Daily Post-Athenian Birth Announcement Form

NEW ARRIVALS are published in The DPA as room permits. Photos may be submitted along 
with the newborn's release, but only the photo of the infant will be used. The photo 
must accompany the birth release. Identifying information must be on the back of the 
photo and all information must be clearly Printed or Typed -- if we can't read it, we 
can't use it. Items appearing on the Lifestyle page will be written in Associated 
Press style.

If parents are not married, please use this form which must be brought to the office 
and both parents must sign the form as well as a member of hospital staff.

Parents have 30 days from the date of the baby's birth to turn in the photo. After 30 days the 
announcement may be published without the photo. Forms submitted after 90 days require a $15 fee. 
Birth Announcements submitted after 6 months will not be published. The DPA will return only those photos submitted 
with a self-addressed stamped envelope. A photo can also be picked up after it has been published
